List of Documents Required to Claim UIF

  1. UI-19 Form: This form is completed by your employer. It details your employment history and the reason for termination of service. Ensure it’s filled out accurately.
  2. ID Document: A certified copy of your South African ID book or card. If you’re a foreign national, you’ll need your passport and a certified copy of your work permit.
  3. Banking Details: Proof of your banking details, usually in the form of a bank statement not older than three months. This is where your UIF payments will be deposited.
  4. Salary Slips: Provide copies of your last six payslips. This helps in calculating the amount you’re entitled to receive.
  5. Service Certificate: A document from your employer confirming your dates of employment and earnings. This certificate is different from the UI-19 form and provides additional employment details.
  6. Proof of Registration: If you’ve already registered for UIF, bring along your UIF registration number or any related documents.
  7. Claim Form: Complete the relevant UIF claim form, which varies based on your claim type (e.g., unemployment, maternity, etc.). You can get these forms at a UIF office or download them from the Department of Labour website.
  8. Medical Certificate: For illness benefits, you’ll need a medical certificate from your doctor.
  9. Birth Certificate of Child: In the case of maternity benefits, provide a certified copy of your child’s birth certificate.
  10. Other Relevant Documents: Depending on your specific situation, additional documents may be required. For example, in the case of adoption, legal documentation confirming the adoption would be necessary.

What about claiming an illness benefit?
If you are sent home to self-quarantine for 14 days as a result of a positive COVID-19 diagnosis and you are unable to work from home, you are entitled to apply for an illness benefit for that specific period.

These are the forms and documents you need to complete and submit:

  • UI-19 and UI-2.7 (completed your employer)
  • UI-2.2 (a portion of which needs to be completed by a medical doctor)
  • UI-2.8 (completed by your bank)
  • Copy of ID document
  • Employer Declaration (Covid19)
  • Employee Declaration (Covid19)

What do I need to claim for a death benefit?
If tragedy strikes and a breadwinner is lost, the families of employees who have passed away are also eligible to apply for some financial relief.
The required documents are:

  • UI-19 and UI-53 (completed by the deceased’s employer)
  • UI-2.5 or UI-2.6
  • Death Certificate
  • ID of deceased and applicant
  • UI-2.8 (bank form completed by the bank)
  • Copy of ID document

 

Are there any other financial relief benefit options?

  9. Payments: UIF payments are not made in lump sums; they are paid out monthly and are subject to the maximum UIF payment thresholds.
